About the role

Join a dedicated healthcare team as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).

Responsibilities

• Performing comprehensive in-home health assessments, including medical history, medication review, vital signs, and physical exams
• Identifying and documenting diagnoses for care management and treatment planning
• Collaborating with primary care providers by communicating findings and addressing care gaps
• Recognizing urgent situations and intervening appropriately
• Educating patients on disease management, medications, and adherence
• Addressing social determinants of health and providing referrals as needed

Qualifications

• Active, unrestricted licensure in the practicing state
• For NPs, active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Valid driver's license and access to reliable transportation
• Ability to navigate varied home environments and perform assessments

Preferred Skills

• 1+ years of clinical experience, especially in geriatrics or home health
• Proficiency with electronic health records and technology
• Physical ability to transport equipment and navigate stairs
• Strong communication skills with geriatric or Medicare populations

Schedule

Flexible scheduling, no on-call, weekends, or holidays, with a focus on preventive care to support patient well-being at home.

Pay

Compensation ranges from $109,500 to $164,000, including benefits and incentives, based on experience and performance.
