LPN Private Duty

About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and/or chronically ill patients in their homes. Works under the supervision of a Registered Nurse or supervisor to deliver high-quality clinical services, document patient visits, and coordinate care activities to ensure optimal patient outcomes.

Responsibilities

• Deliver nursing care within legal scope and plan of care, adhering to infection control protocols.
• Accurately document patient visits and progress in electronic medical records and visit verification systems.
• Participate in care coordination, report patient needs and changes, and communicate effectively with patients, families, physicians, and team members.

Requirements

• Current, unrestricted LPN licensure and CPR certification.
• Valid driver’s license, reliable transportation, or access to public transit.
• Ability to work independently and manage multiple tasks.
• Strong communication, organizational, and documentation skills.

Preferred Qualifications

• Home care experience.
• Flexibility to meet client needs.

Pay

Pay ranges from $22.90 to $34.35 per hour, based on experience and market factors.
