About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, under supervision of a Registered Nurse or designated supervisor.

Responsibilities

• Deliver high-quality clinical services within the scope of practice, adhering to state laws and the patient's care plan.
• Document patient visits thoroughly in electronic medical records and visit verification systems.
• Participate actively in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Requirements

• Current and unrestricted LPN licensure in the practice state.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
• Ability to work independently in diverse home environments regardless of client background.
