About the role

Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions and the plan of care. The role emphasizes a culture of caring, connection, and growth, with a focus on equitable, quality patient care.

Responsibilities

• Provide direct nursing care to patients in their homes, adhering to physician prescriptions and the plan of care.
• Document patient visits thoroughly in electronic medical records, ensuring compliance with regulatory and payer requirements.
• Participate in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Requirements

• Current, unrestricted LPN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle.
• Ability to work independently and adapt to various home environments.
• Strong communication, organizational, and multitasking skills.
