LPN Private Duty

About the role

Provide direct nursing care to acutely and chronically ill patients in their homes, under the supervision of a Registered Nurse or designated supervisor. Deliver high-quality clinical services within legal scope, document visits thoroughly in electronic records, and participate in care coordination by communicating patient needs and condition changes effectively with patients, families, physicians, and team members.

Requirements

• Current LPN licensure
• CPR certification
• Valid driver’s license
• Access to transportation

Qualifications

• Home care experience (preferred)
• Ability to work independently and manage multiple tasks
• Strong communication skills
