About the role

Provide direct nursing care to acutely or chronically ill patients in their homes. Coordinate care activities, ensure high-quality care within the scope of practice and state laws, and collaborate with healthcare team members to meet patient needs.

Responsibilities

• Document patient visits and maintain thorough records in electronic medical systems
• Coordinate care activities and report patient needs to the healthcare team
• Ensure high-quality care in compliance with state laws and scope of practice

Requirements

• Current LPN licensure
• CPR certification
• Valid driver’s license and reliable transportation
• Must pass a drug test
