Long Term Care MDS Supervisor

About the role

The Minimum Data Set “MDS” Supervisor takes a comprehensive assessment of each patient’s needs and develops nursing care plans while managing the MDS department. Manages the MDS processes in accordance with government regulations and policies with the focus on maintaining excellent care for patients. Located at Wheeling Hospital Inc., 236 Hullihen Place, Wheeling, West Virginia (Cost Center 553 WH CCC Utilization Review).

Responsibilities

• Assists with the planning organization, development, and coordination of the care planning process upon admission of all new patients
• Plays an active role in maintaining rapport with all departments to ensure a team effort is achieved in developing, implementing, and evaluating care plans for patients.
• Assists in reviewing and evaluating the comprehensive assessment date and helps identify when and what additional assessments are triggered.
• Completes and submits the MDS timely and accurately to ensure every MDS deadline is met.
• Communicates effectively with residents and their families to ensure they are receiving excellent care and their needs are being met throughout their length of stay
