About the role
Provides comprehensive social work services for a defined population, collaborating with healthcare teams to coordinate appropriate, cost-effective care that supports members' independence and quality of life.
Responsibilities
- Conducting psychosocial assessments, problem identification, and providing counseling and referrals.
- Managing caseloads, developing and monitoring individualized care plans in collaboration with healthcare professionals.
- Facilitating access to community resources, legal processes, and funding programs (e.g., Medicaid, Medicare).
- Documenting activities per organizational standards, analyzing outcomes for quality improvement.
- Supporting members, families, and caregivers through education, counseling, and end-of-life planning.
- Scheduling family meetings, completing guardianship paperwork, and providing technology support for court proceedings.
- In inpatient settings, completing assessments for long-term care as per Medicaid requirements.
Qualifications
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensure from multiple states required, including an Independent or Graduate Social Worker license within six months of hire based on location.
Skills
- Experience with Windows-based software programs.
- Knowledge of community resources and regulatory issues in the Mid-Atlantic area.
