About the role
This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for a defined population.
Responsibilities
- Conduct psychosocial assessments, identify problems, and provide counseling, legal guidance, and resource referrals.
- Manage caseloads to develop and monitor individualized care plans aimed at promoting independence and improving quality of life.
- Advocate for access to community resources and ensure timely, cost-effective implementation.
- Document activities in compliance with standards and analyze outcomes for continuous improvement.
- Assist with applications for programs like Medicaid and Medicare, and support members, families, and caregivers, including end-of-life planning.
- Schedule family meetings, complete guardianship paperwork, and provide technological support for court proceedings.
- Perform assessments for long-term care as required for inpatient settings.
Qualifications
- Minimum of one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licenses from multiple states, including requirements for DC, Maryland, and Virginia within six months of hire.
Skills
Additional skills and knowledge in community systems, resources, and regulatory issues are preferred.
