About the role
We are seeking a Social Worker responsible for coordinating patient care and facilitating access to resources for a defined population.
Responsibilities
- Perform psychosocial assessments, problem identification, and provide counseling and referrals.
- Collaborate with healthcare team members to develop and monitor individualized care plans aimed at optimizing independence and quality of life.
- Advocate for appropriate community and organizational resources, ensuring timely and cost-effective service delivery.
- Document case management activities in compliance with standards.
- Analyze outcomes to improve program quality and efficiency.
- Assist with applications for government-funded programs and handle legal processes like guardianship and court attendance.
- Provide education and support to members, families, and healthcare professionals, including end-of-life planning.
- Maintain knowledge of laws, policies, and standards related to social work practice.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master's degree in social work (MSW).
- Licenses from relevant states within six months of hire, including Licensed Independent or Graduate Social Worker, and Licensed Master Social Worker in Maryland and Virginia.
Additional Details
- Training provided for cross-covering Baltimore hospitals.
- Requires working every other weekend and holidays.
Preferred Skills
- Familiarity with community resources, regulatory issues, and Windows-based software.
