LMSW-Case Manager, Social Work - Inpatient (Part-Time)

About the role

Responsible for collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with members' care plans, promoting independence, and improving quality of life.

Responsibilities

• Conduct psychosocial assessments, identify problems, provide financial counseling, access community resources, and support legal and care placement processes.
• Manage and coordinate caseloads, developing plans to optimize member functioning and quality of life.
• Advocate for and facilitate access to community and organizational resources, ensuring timely and efficient implementation.
• Document activities per organizational standards and analyze outcomes to improve program effectiveness and cost efficiency.
• Assist with applications for public assistance programs and refer suspected cases of abuse or neglect.
• Provide counseling, education, and support to members, families, caregivers, and healthcare teams, including end-of-life discussions and advance directives.
• Maintain current knowledge of social work standards, laws, and policies, and coordinate family meetings and court-related assistance as needed.
• Complete assessments for long-term care as required and perform other duties as assigned.

Requirements

• Minimum of 1 year of clinical social work experience in healthcare.
• Master's degree in social work (MSW).
• Licensing credentials for multiple states, including required licenses within 6 months of hire.

Preferred Qualifications

• Knowledge of community resources, regulatory issues, and computer software in a Windows environment.

Pay
