LMSW - Case Manager, Social Work - Inpatient (Part-Time)

About the role

Coordinate patient care within a healthcare team to improve outcomes, independence, and quality of life.

Responsibilities

• Perform psychosocial assessments, identify problems, and provide counseling and resource referrals.
• Manage a caseload, develop and monitor individualized care plans in collaboration with healthcare professionals.
• Facilitate access to community, organizational, and government resources, ensuring timely and cost-effective service delivery.
• Document activities in compliance with organizational standards and analyze outcomes to improve program efficiency.
• Assist with applications for government programs and provide education on legal and end-of-life issues.
• Maintain current knowledge of social work standards, laws, and regulations.
• Schedule family meetings as needed.
• Complete guardianship paperwork and support virtual court attendance when required.

Requirements

• Minimum one year of clinical social work experience in a healthcare setting.
• Master’s degree in social work (MSW).
• Multistate credentials, with licensing within six months in applicable states.

Skills

• Experience with Windows-based software and community resources.
• Knowledge of regional regulatory issues.
