About the role
This role involves coordinating comprehensive social work services for a designated patient population, collaborating with healthcare teams to develop and monitor individualized care plans that promote independence and improve quality of life.
Responsibilities
- Conduct psychosocial assessments, identify problems, and provide counseling and support to patients, families, and healthcare professionals.
- Access community resources, assist with legal and financial processes, and facilitate patient placements.
- Develop and track care plans to optimize patient functioning and resource utilization.
- Document activities accurately, analyze outcomes to improve programs, and advocate for appropriate services, including Medicaid, Medicare, and disability programs.
- Provide education on end-of-life issues, advanced directives, and community resources, and coordinate family meetings and guardianship paperwork.
- Perform inpatient assessments, including long-term care forms, and support virtual court proceedings as needed.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master's degree in social work (MSW).
- Licensing credentials from multiple states required within specified timeframes.
Skills
- Experience with Windows-based software.
- Knowledge of local community systems and awareness of regional regulatory issues are preferred.
