LMSW Case Manager, Social Work - Inpatient (Part-Time)

About the role

This role involves coordinating care for members of a defined population to promote independence and enhance quality of life. The position includes conducting psychosocial assessments, providing counseling, facilitating access to community resources, and supporting legal and care placement processes.

Responsibilities

• Manage a caseload and develop and monitor individualized care plans.
• Collaborate with healthcare team members to optimize patient outcomes.
• Advocate for appropriate services and ensure proper documentation.
• Analyze program effectiveness to identify improvements.
• Assist with application processes for government programs.
• Provide family and member education.
• Maintain current knowledge of social work standards and regulations.
• Train to cross cover Baltimore hospitals and work every other weekend and holiday shifts:
  • Week 1: Sun, Wed, Thurs
  • Week 2: Thurs, Fri, Sat

Qualifications

• Minimum one year of clinical social work experience in healthcare.
• Master’s degree in social work (MSW).
• Licensure as an Independent or Graduate Social Worker in the primary state at hire, with additional state credentials required subsequently.
• Licenses in DC, Maryland, and Virginia within six months of hire.

Preferred Skills

• Experience with Windows-based software programs.
• Knowledge of community resources and regulatory issues in the Mid-Atlantic region.
