LMSW-Case Manager, Social Work - Inpatient (Part-Time)

Responsibilities

  • Collaborate with healthcare team to coordinate cost-effective, appropriate services aligned with members' care plans.
  • Conduct psychosocial assessments, identify problems, and provide counseling, referrals, and resource access.
  • Develop, implement, and monitor individualized care plans to promote optimal functioning and quality of life.
  • Advocate for and facilitate access to community, organizational, and government resources, ensuring timely and efficient care.
  • Document activities consistently, analyze outcomes, and seek continuous improvement in program quality and cost efficiency.
  • Assist members with applying for public programs (e.g., Medicaid, Medicare), and report suspected abuse/neglect.
  • Provide education and support to members, families, caregivers, and healthcare professionals, including end-of-life planning.
  • Maintain current knowledge of laws, regulations, and best practices in social work, adhering to NASW standards.
  • Coordinate family meetings, complete guardianship paperwork, and assist with virtual court proceedings as needed.
  • For inpatient roles, complete assessments required for long-term care eligibility.
  • Perform additional duties as assigned.

Qualifications

  • Minimum of 1 year of clinical social work experience in healthcare.
  • Master’s degree in social work (MSW).
  • Licenses from multiple states required, including Licensed Independent Social Worker or Licensed Graduate Social Worker (DC), and Licensed Master Social Worker (MD, VA) within 6 months of hire.

Skills

  • Familiarity with community resources, regulatory issues, and computer software in a Windows environment (preferred).
