Responsibilities
- Collaborate with healthcare team to coordinate cost-effective, appropriate services aligned with members' care plans.
- Conduct psychosocial assessments, identify problems, and provide counseling, referrals, and resource access.
- Develop, implement, and monitor individualized care plans to promote optimal functioning and quality of life.
- Advocate for and facilitate access to community, organizational, and government resources, ensuring timely and efficient care.
- Document activities consistently, analyze outcomes, and seek continuous improvement in program quality and cost efficiency.
- Assist members with applying for public programs (e.g., Medicaid, Medicare), and report suspected abuse/neglect.
- Provide education and support to members, families, caregivers, and healthcare professionals, including end-of-life planning.
- Maintain current knowledge of laws, regulations, and best practices in social work, adhering to NASW standards.
- Coordinate family meetings, complete guardianship paperwork, and assist with virtual court proceedings as needed.
- For inpatient roles, complete assessments required for long-term care eligibility.
- Perform additional duties as assigned.
Qualifications
- Minimum of 1 year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licenses from multiple states required, including Licensed Independent Social Worker or Licensed Graduate Social Worker (DC), and Licensed Master Social Worker (MD, VA) within 6 months of hire.
Skills
- Familiarity with community resources, regulatory issues, and computer software in a Windows environment (preferred).
