About the Role
Responsible for collaborating with healthcare team members to coordinate cost-effective services aligned with members' care plans, aiming to improve independence and quality of life.
Responsibilities
- Perform psychosocial assessments, identify problems, provide financial counseling, access community resources, and support legal processes for health-related issues.
- Manage and coordinate caseloads, develop and monitor care plans to optimize functioning and enhance quality of life.
- Facilitate access to community and organizational resources, ensuring timely and cost-effective implementation.
- Document activities in compliance with standards, analyze outcomes for improvements, and promote best practices in case management.
- Assist with applications for funded programs, provide counseling and education to members and families, and coordinate family meetings.
- Complete legal and assessment documentation, including guardianship and court-related support, especially for inpatient cases.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensure from multiple states, including requirements for D.C., Maryland, and Virginia, with specified licensing within six months of hire.
Skills
- Preferred familiarity with computer software, community systems, and regional regulatory issues.
