About the role
Responsible for collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with members' care plans, aiming to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments, problem-solving, financial counseling, community resource access, and legal process guidance.
- Manage caseloads, evaluate patient goals, support systems, health status, and environmental factors to optimize care and reduce unnecessary medical utilization.
- Develop and monitor care plans with healthcare team members to promote functioning and well-being.
- Advocate for and facilitate access to community and organizational resources, ensuring efficient implementation.
- Maintain thorough documentation of case management activities in compliance with standards.
- Analyze outcomes to improve program quality, efficiency, and cost-effectiveness.
- Assist members with applications for public programs and refer suspected abuse cases to appropriate agencies.
- Provide counseling and education to members, families, caregivers, and healthcare teams, including end-of-life care and directives.
- Stay current on case management practices, laws, and regulations, adhering to social work standards.
- Coordinate family meetings, complete guardianship paperwork, and support virtual court attendance as needed.
- Perform inpatient-specific tasks such as completing assessment forms for long-term care.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licenses required for multiple states, including either Licensed Independent Social Worker or Licensed Graduate Social Worker within 6 months of hire, and state-specific licenses (Maryland, Virginia) within 6 months.
Preferred Skills
- Experience with Windows-based software and knowledge of local community resources.
- Understanding of regulatory issues in the Mid-Atlantic region.
