About the role
Coordinate cost-effective, appropriate services for members to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments, identify problems, and provide counseling and support to members, families, and healthcare teams.
- Develop and monitor individualized plans of care to promote optimal functioning and enhance life quality.
- Facilitate access to community resources, legal processes, and funding programs like Medicaid and Medicare.
- Document activities in compliance with standards, analyze outcomes for program improvement, and ensure efficient resource utilization.
- Provide education on end-of-life issues, advanced directives, and community resources.
- Coordinate family meetings, complete guardianship paperwork, and assist with virtual court proceedings when needed.
- Perform assessments for long-term care as required and support inpatient and hospital coverage, including holiday shifts.
Qualifications
- Minimum one year of clinical social work experience in healthcare.
- Master's degree in social work (MSW) required.
- Licensure from multiple states or eligibility within specified timeframes, including LICSW or equivalent credentials.
Skills
- Experience with Windows-based software.
- Knowledge of local community resources.
- Understanding of regulatory issues in the Mid-Atlantic region.
