LMSW-Case Manager, Social Work - Inpatient (12 hrs.)

About the role

Coordinate cost-effective, appropriate services for members to improve independence and quality of life.

Responsibilities

• Conduct psychosocial assessments, identify problems, and provide counseling and support to members, families, and healthcare teams.
• Develop and monitor individualized plans of care to promote optimal functioning and enhance life quality.
• Facilitate access to community resources, legal processes, and funding programs like Medicaid and Medicare.
• Document activities in compliance with standards, analyze outcomes for program improvement, and ensure efficient resource utilization.
• Provide education on end-of-life issues, advanced directives, and community resources.
• Coordinate family meetings, complete guardianship paperwork, and assist with virtual court proceedings when needed.
• Perform assessments for long-term care as required and support inpatient and hospital coverage, including holiday shifts.

Qualifications

• Minimum one year of clinical social work experience in healthcare.
• Master's degree in social work (MSW) required.
• Licensure from multiple states or eligibility within specified timeframes, including LICSW or equivalent credentials.

Skills

• Experience with Windows-based software.
• Knowledge of local community resources.
• Understanding of regulatory issues in the Mid-Atlantic region.
