About the Role
This position involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments, problem identification, and provide counseling and support to members, families, and caregivers.
- Develop and monitor individualized care plans in partnership with healthcare professionals.
- Facilitate access to community resources and assist with applications for programs such as Medicaid and Medicare.
- Ensure accurate documentation and analyze outcomes to enhance program quality and cost efficiency.
- Provide education on legal, health-related, and end-of-life issues, and coordinate family meetings and guardianship paperwork.
- Support inpatient activities, including completing assessments for long-term care as per Medicaid requirements.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Required licensing or credentials in multiple states, including licenses from D.C., Maryland, and Virginia within six months of hire.
Skills
- Preferred: familiarity with community resources, regulatory issues, and computer software in a Windows environment.
