LMSW-Case Manager, Social Work - Inpatient

About the role

This role involves providing psychosocial support and care coordination for a defined patient population.

Responsibilities

• Conduct psychosocial assessments, problem identification, and provide counseling and education to members, families, and caregivers.
• Collaborate with healthcare team members to develop and monitor personalized care plans aimed at enhancing members’ independence and quality of life.
• Facilitate access to community resources, legal processes, and funding programs like Medicaid and Medicare.
• Document activities accurately in compliance with organizational standards and analyze outcomes for continuous improvement.
• Perform related duties such as scheduling family meetings, completing guardianship paperwork, and providing virtual court attendance support.

Requirements

• Minimum one year of clinical social work experience in healthcare.
• Master’s degree in social work (MSW).
• Licensure as an Independent or Graduate Social Worker in relevant states within six months of hire.

Skills

• Experience with Windows-based software and knowledge of community resources.
• Understanding of regional regulatory issues.
