LMSW - Case Manager, Social Work - Inpatient (Full-Time)

This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life.

Responsibilities

• Conduct psychosocial assessments
• Develop and monitor care plans
• Facilitate access to community resources
• Provide supportive counseling to members, families, and caregivers
• Manage caseloads and document activities
• Analyze outcomes to improve programs
• Assist with applications for community and government programs
• Complete assessments and guardianship paperwork for inpatient duties

Requirements

• Master’s degree in social work
• At least one year of clinical social work experience
• Licensure in relevant states within six months of hire

Preferred Skills

• Proficiency with Windows-based software
• Knowledge of community resources
• Familiarity with regional regulatory issues
