About the role
This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments, identify problems, provide counseling, and refer to community resources.
- Manage a caseload, develop and monitor care plans to promote optimal functioning.
- Advocate for resources and ensure timely, cost-effective implementation.
- Document activities accurately and analyze outcomes for quality improvements.
- Assist with applications for government programs and address issues like abuse or neglect.
- Provide education and support to members, families, and healthcare staff, including end-of-life planning.
- Maintain current knowledge of laws, regulations, and standards of social work practice.
- Schedule family meetings, complete guardianship paperwork, and assist with technology for court proceedings.
- Perform inpatient assessments related to long-term care as required.
Qualifications
- Minimum one year of clinical social work experience in healthcare.
- A master's degree in social work (MSW).
- Licenses or credentials required from multiple states, with specific licensing timelines.
Skills
- Preferred knowledge of community resources and regulatory issues.
- Experience with computer software.
