About the role

This position involves collaborating with healthcare team members to coordinate cost-effective, appropriate services that align with members' care plans, aiming to improve independence and quality of life.

Responsibilities

  • Conduct psychosocial assessments, problem-solving, financial counseling, and access to community resources.
  • Support legal health processes and manage caseloads.
  • Develop and monitor care plans to optimize member functioning and well-being.
  • Advocate for resources, ensure timely implementation, and document activities per standards.
  • Analyze outcomes to improve program quality and cost efficiency.
  • Assist members with application processes for public programs and address abuse/neglect cases.
  • Provide counseling, education, and support to members, families, caregivers, and healthcare staff, including end-of-life discussions and advanced directives.
  • Maintain current knowledge of social work standards, laws, and regulations.
  • Coordinate family meetings and guardianship paperwork.
  • Perform related duties, including inpatient assessments and court support as needed.

Requirements

  • Minimum of one year of clinical social work experience in healthcare.
  • Master’s degree in social work (MSW).
  • Licenses from multiple states are required, including credentials within six months of hire for specific licenses.

Skills

  • Experience with Windows-based software and community resources.
  • Knowledge of regulatory issues in the Mid-Atlantic region.
