LMSW Case Manager, Social Work - Full-Time

About the role

This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life.

Responsibilities

• Conduct psychosocial assessments of members.
• Develop, implement, and monitor individualized care plans.
• Advocate for and connect members with community resources.
• Ensure proper documentation and compliance with regulatory requirements.
• Provide counseling, education, and support to members, families, and caregivers.
• Facilitate access to public assistance programs.
• Complete inpatient assessments and guardianship paperwork as needed.

Requirements

• Master’s degree in social work (MSW).
• At least one year of clinical social work experience in a healthcare setting.
• Licensure in relevant states.
• Flexibility to work weekends as required.

Preferred Qualifications

• Knowledge of community resources and regulatory issues in healthcare.
• Proficiency with computer software and electronic documentation systems.
