LMSW Case Manager, Social Work - Full-Time

About the role

Responsible for collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with members' care plans, promoting independence, and improving quality of life for a defined population.

Responsibilities

• Conducts psychosocial assessments, identifies problems, and provides financial counseling, community resource access, legal support, and professional consultation.
• Manages a caseload, develops and monitors care plans to enhance members' functioning and quality of life.
• Advocates for community resources, ensures timely implementation, and documents activities per organizational standards.
• Analyzes outcomes to improve program quality and cost-effectiveness.
• Assists members with accessing public programs, reports suspected abuse, and provides counseling and education to members and families.
• Maintains current knowledge of regulations, promotes best practices, and coordinates family meetings and court-related support.
• Completes necessary assessments and documentation, performs related duties as assigned.

Requirements

• Minimum one year of clinical social work experience in healthcare.
• Master's degree in social work (MSW).
• Licensure in relevant states within six months of hire, including Licensed Independent or Graduate Social Worker, or Licensed Master Social Worker, depending on location.

Preferred Qualifications

• Experience with Windows-based software.
• Knowledge of community resources and regulatory issues in the service area.
