About the Role
Responsible for collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with members' care plans, aiming to promote independence and improve quality of life.
Responsibilities
- Conduct psychosocial assessments and support access to resources.
- Provide guidance on legal processes and support healthcare professionals.
- Manage caseloads, develop and monitor care plans, and advocate for community resources.
- Ensure proper documentation and analyze outcomes to improve program effectiveness.
- Facilitate access to funded programs and provide counseling and education to members, families, and staff.
- Promote awareness of current standards, legal regulations, and best practices in social work.
- Schedule family meetings and complete guardianship paperwork.
- Support virtual court attendance.
- Complete assessment forms for long-term care in inpatient roles as required.
- Cross-cover inpatient and hospital settings, including holiday coverage.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensing credentials from multiple states required; specific licenses needed within six months of hire.
Skills
- Familiarity with Windows-based software.
- Knowledge of community resources and regulatory issues in the Mid-Atlantic region.
