LMSW-Case Manager, Social Work - Inpatient (12 hrs.)

About the Role

Responsible for collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with members' care plans, aiming to promote independence and improve quality of life.

Responsibilities

• Conduct psychosocial assessments and support access to resources.
• Provide guidance on legal processes and support healthcare professionals.
• Manage caseloads, develop and monitor care plans, and advocate for community resources.
• Ensure proper documentation and analyze outcomes to improve program effectiveness.
• Facilitate access to funded programs and provide counseling and education to members, families, and staff.
• Promote awareness of current standards, legal regulations, and best practices in social work.
• Schedule family meetings and complete guardianship paperwork.
• Support virtual court attendance.
• Complete assessment forms for long-term care in inpatient roles as required.
• Cross-cover inpatient and hospital settings, including holiday coverage.

Requirements

• Minimum one year of clinical social work experience in healthcare.
• Master’s degree in social work (MSW).
• Licensing credentials from multiple states required; specific licenses needed within six months of hire.

Skills

• Familiarity with Windows-based software.
• Knowledge of community resources and regulatory issues in the Mid-Atlantic region.
