LMSW Case Manager - Inpatient Social Work (24hrs.)

About the role

This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life.

Responsibilities

• Conduct psychosocial assessments to identify member needs.
• Manage caseloads, develop and monitor individualized care plans.
• Advocate for community and organizational resources to support members.
• Document all case management activities and analyze outcomes for quality and cost improvements.
• Support members through legal processes and facilitate access to necessary resources.
• Complete inpatient assessments for long-term care and facilitate court proceedings as required.
• Provide counseling, education, and family coordination.
• Adhere to current case management standards and legal regulations.

Requirements

• Master’s degree in social work.
• At least one year of clinical social work experience.
• Must obtain multi-state licensure within six months of hire.

Preferred Qualifications

• Experience with community resources and regulatory issues.
• Proficiency with computer software for case management and documentation.
