About the role
This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments to identify member needs.
- Manage caseloads, develop and monitor individualized care plans.
- Advocate for community and organizational resources to support members.
- Document all case management activities and analyze outcomes for quality and cost improvements.
- Support members through legal processes and facilitate access to necessary resources.
- Complete inpatient assessments for long-term care and facilitate court proceedings as required.
- Provide counseling, education, and family coordination.
- Adhere to current case management standards and legal regulations.
Requirements
- Master’s degree in social work.
- At least one year of clinical social work experience.
- Must obtain multi-state licensure within six months of hire.
Preferred Qualifications
- Experience with community resources and regulatory issues.
- Proficiency with computer software for case management and documentation.
