LMSW-Case Manager, Social Work - Inpatient (12 hrs.)

About the role

The social worker collaborates with healthcare teams to coordinate cost-effective, appropriate services aligned with members' care plans. The role aims to promote members' independence and improve quality of life by addressing psychosocial needs and facilitating access to resources.

Responsibilities

• Conduct psychosocial assessments, identify problems, provide counseling, and refer members to community resources or legal services.
• Manage and coordinate caseloads, develop and monitor individualized care plans to enhance functioning and quality of life.
• Advocate for resources, ensure timely implementation, and document activities in compliance with standards.
• Analyze outcomes to improve program efficiency, quality, and cost-effectiveness.
• Assist members with applications for government-funded programs and address issues like abuse or neglect.
• Provide education and support to members, families, caregivers, and healthcare staff, including end-of-life planning.
• Maintain current knowledge of laws, regulations, and best practices, and facilitate family meetings and court-related processes as needed.
• Complete assessments for long-term care and perform other duties as assigned.

Qualifications

• Minimum of one year of clinical social work experience in healthcare.
• Master's degree in social work (MSW).
• Licenses from multiple states required, including within six months of hire (e.g., Licensed Independent or Graduate Social Worker, Licensed Master Social Worker).

Skills

• Preferred experience with computer software, community systems, and knowledge of regulatory issues in the Mid-Atlantic region.

Additional Notes

• Cross-cover inpatient and hospital settings, with holiday coverage required.
• Inpatient duties include completing assessments and guardianship paperwork, and providing technology assistance for court proceedings.
