LMSW Case Manager - Inpatient Social Work (24 hrs.)

About the Role

This position involves collaborating with the healthcare team to coordinate appropriate, cost-effective services for members, aiming to enhance independence and quality of life.

Responsibilities

• Conduct psychosocial assessments and develop care plans.
• Manage caseloads and facilitate access to community and organizational resources.
• Provide counseling, education, and support to members, families, caregivers, and healthcare professionals.
• Ensure accurate documentation and analyze outcomes to improve program effectiveness.
• Assist with legal and financial processes, including scheduling family meetings and completing guardianship paperwork.
• Support inpatient assessments for long-term care.

Requirements

• Master’s degree in social work.
• At least one year of clinical social work experience in healthcare.
• Relevant licensure or credentials in multiple states.

Preferred Skills

• Familiarity with community resources and regulatory issues.
• Proficiency with computer software in a Windows environment.
