About the Role
The Care Coordinator applies clinical expertise to manage the care of primarily medically complex members, ensuring access to appropriate services across various settings.
Responsibilities
- Conduct assessments and develop individualized care plans.
- Provide patient education and facilitate community resource referrals.
- Collaborate with multidisciplinary teams to optimize care delivery and reduce hospitalizations.
- Engage in telephonic and face-to-face interactions with members.
- Document care activities and advocate to address barriers to care.
- Ensure compliance with regulatory standards.
Requirements
- Active nursing (RN) or social work license.
- At least 2 years of clinical experience.
- Proficiency in MS Office.
- Ability to travel within designated counties.
Qualifications (Preferred)
- Higher degrees in nursing or social work.
- Certifications such as CCM (Certified Case Manager).
- Experience with complex care coordination.
- Knowledge of Medicare/Medicaid regulations.
- Experience with team-based care models.
