About the Role

The Care Coordinator applies clinical expertise to manage the care of primarily medically complex members, ensuring access to appropriate services across various settings.

Responsibilities

  • Conduct assessments and develop individualized care plans.
  • Provide patient education and facilitate community resource referrals.
  • Collaborate with multidisciplinary teams to optimize care delivery and reduce hospitalizations.
  • Engage in telephonic and face-to-face interactions with members.
  • Document care activities and advocate to address barriers to care.
  • Ensure compliance with regulatory standards.

Requirements

  • Active nursing (RN) or social work license.
  • At least 2 years of clinical experience.
  • Proficiency in MS Office.
  • Ability to travel within designated counties.

Qualifications (Preferred)

  • Higher degrees in nursing or social work.
  • Certifications such as CCM (Certified Case Manager).
  • Experience with complex care coordination.
  • Knowledge of Medicare/Medicaid regulations.
  • Experience with team-based care models.
