Licensed Field Care Coordinator

About the Role

The Care Coordinator applies clinical expertise to manage the care of medically complex members, ensuring access to appropriate services across various settings. This field-based position involves collaborating with multidisciplinary teams, developing personalized care plans, providing education on self-management and community resources, conducting assessments, facilitating care transitions, and monitoring hospital encounters to reduce readmissions. The role emphasizes member advocacy, documentation compliance, and participation in quality improvement initiatives.

Requirements

• Current RN license or Licensed Social Worker
• At least 2 years of clinical experience
• Proficiency in MS Office
• Valid driver’s license and reliable transportation for travel within assigned territory
• Reside within a 50-mile radius of specified counties

Preferred Qualifications

• BSN, Master’s degree, or higher in clinical field
• CCM certification
• Experience in community case management and long-term care
• Knowledge of Medicare/Medicaid regulations and medical terminology
• Strong organizational, communication, and time management skills
