RN Field Clinical Care Coordinator - Full-Time

Responsibilities

• Conduct comprehensive needs assessments face-to-face and via phone
• Develop and implement person-centered care plans for managing chronic conditions, health promotion, and social determinants
• Collaborate with care teams, providers, and community resources
• Provide education and coaching to support member self-management
• Coordinate discharge planning and care transitions after ER visits or hospital stays
• Advocate for members to ensure their needs and preferences are addressed

Qualifications & Skills

• High School Diploma/GED and RN licensure in MA
• 2+ years of clinical experience
• Proficiency in MS Office
• Reliable home workspace with PHI security
• Valid US driver’s license and access to transportation

Preferred:

• Bachelor's/Master’s in Nursing
• Care Manager certification
• Community case management experience
• Familiarity with electronic medical records
• Familiarity with virtual care platforms
