About the role

The Field Care Coordinator is responsible for assessing and developing personalized care plans for enrollees, promoting their independence and self-sufficiency. This role involves conducting in-depth assessments in members' homes, nursing facilities, and care settings, and collaborating with interdisciplinary teams to ensure comprehensive, cost-effective care.

Responsibilities

• Managing care transitions

• Providing education on self-management
• Coordinating with providers and community resources

Qualifications

• Current license as an LPN or a related field with a 4-year degree
• At least 3 years of care coordination or healthcare experience
• Proficiency in MS Office
• Reliable transportation

Skills

• CCM certification (preferred)
• Experience with Medicaid/Medicare populations (preferred)
• Geriatric care knowledge (preferred)

Physical Requirements

The role requires mobility, transportation of equipment, and ability to work in various locations.
