This position requires 75-90% local travel to visit Medicaid members in their homes, Assisted Living Facilities, Long Term Care Facilities, and other care settings.

Responsibilities

  • Assess and evaluate members' needs to establish a member-specific care plan.
  • Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well-being.
  • Plan and implement interventions to meet members' needs.
  • Coordinate services and monitor/evaluate the case management plan against the member’s personal goals.
  • Guide members and families toward appropriate resources for their care.
  • Facilitate interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member’s care as required.

Requirements

  • Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology), or
  • Registered Nurse (RN), licensed in the state of Florida, with at least 2 years of relevant experience, or
  • Licensed Practical Nurse (LPN), licensed in the state of Florida, with at least 4 years of relevant experience, or
  • Case Worker with a minimum of 6 years of Health Services/Case Management experience.
  • Prior experience with Medicare & Medicaid recipients.
  • Intermediate to advanced computer skills, including Microsoft Word, Excel, and Outlook.
  • AHCA Fingerprint required.
  • This role is patient-facing and requires Tuberculosis (TB) screening.
  • Valid state driver’s license and proof of personal vehicle liability insurance.
  • Ability to work Monday-Friday, 8:30 AM - 5:00 PM.

Preferred Qualifications

  • Bilingual (English/Spanish) with ability to pass a Language Proficiency Exam.
  • Community Health Worker experience.
  • Prior nursing home diversion or long-term care case management experience.
  • Experience with electronic case note documentation and multiple computer applications/systems.
  • Experience working with the geriatric population.
  • Experience with health promotion, coaching, and wellness.
  • Knowledge of community health and social service agencies and resources.
  • 701B Certification.
  • Medication application (RFA) experience.

Pay

The starting salary for this position is $64,000 - $68,000 per year based on qualifications and experience. The full pay range for this role is $53,700 - $72,600 per year.

Schedule

This is a full-time position with scheduled weekly hours of 40, Monday through Friday, 8:30 AM - 5:00 PM.

This role is based in the Boca Raton, Delray, and Boynton Beach areas of Florida. Candidates must reside in or be willing to relocate to these areas.
