Qualifications & Skills
- Licensed Practical Nurse (Virginia) or related degree in Social Work/Human Services with 3+ years of care coordination or healthcare experience
- Experience with medical needs, elderly, disabled populations, and communication barriers
- Proficiency in MS Office
- Reliable transportation for travel within the territory
- PREFERRED: CCM certification, Medicaid/Medicare experience, geriatric or long-term care background
Key Duties
- Conduct face-to-face or telephonic needs assessments
- Develop and implement personalized care plans
- Cook up with care teams and community resources
- Coordinate support for member education and self-management
- Aid with discharge planning and care transitions
- Advocate for members' needs and preferences
