Inpatient Social Worker Case Manager, Care Coordination (Full Time)

Responsibilities

• Coordinates and implements case management activities for patients during their stay in the hospital.
• Remains the primary point of contact for patients, families, and legal representatives until they transition to other service lines or case managers.
• Reviews and assesses the appropriateness of care and treatment against screening and reimbursement criteria, and makes referrals and resource requests as needed.
• Sends necessary referrals to appropriate service line Patient Navigators, OP Case Managers, or community resource contacts for review.
• Attends daily care rounds and communicates any changes in the patient's clinical condition to the multi-disciplinary care team.
• Communicates and collaborates with multidisciplinary care team members, Patient Navigators, OP Case Managers, and patients/families/legal representatives.
• Manages and modifies patient care plans as necessary based on ongoing assessments and clinical updates.
• Deals with pressure to meet deadlines, maintains accuracy, and handles constantly changing situations.
• Handles various personalities and situations, including stressful and conflictual ones.

Requirements

• Bachelor of Social Work (BSW) preferred.
• At least 2 years of medical/hospital experience.
• At least 2 years of case management experience preferred.

Qualifications

• Must comply with all Benefis Health System’s organization policies and procedures.
