About the role
The social worker is responsible for collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with the member's care plan, aiming to promote independence and improve quality of life.
Responsibilities
- Conduct psychosocial assessments, identify problems, provide counseling, and facilitate access to community resources.
- Manage and coordinate caseloads, develop and monitor care plans to enhance member functioning.
- Advocate for resources, ensure documentation compliance, and analyze outcomes for program improvement.
- Assist with applications for funded programs, provide education on end-of-life issues, and support legal processes.
- Maintain current knowledge of laws, regulations, and standards, and coordinate family meetings and guardianship paperwork as needed.
Qualifications
- Minimum one year of clinical social work experience in healthcare.
- Master's degree in social work (MSW).
- Licensure as an Independent or Graduate Social Worker in the primary state, with additional credentials required for other states.
Skills
- Knowledge of community resources, regulatory issues, and experience with computer software preferred.
