About the role
This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services that align with members' care plans, promoting independence and improving quality of life.
Responsibilities
- Conducting psychosocial assessments, identifying problems, providing financial counseling, accessing community resources, and supporting legal processes.
- Managing caseloads by developing and monitoring individualized care plans in collaboration with healthcare team members.
- Facilitating access to community and organizational resources, ensuring timely and cost-effective implementation.
- Documenting activities consistently per standards, analyzing outcomes for program improvement, and advocating for patients’ needs.
- Assisting with applications for programs like Medicaid, Medicare, and Disability, and providing counseling and education to members, families, and caregivers.
- Promoting current case management standards, maintaining knowledge of laws and policies, and coordinating family meetings and court-related support.
- In inpatient settings, completing assessments for long-term care as required.
Qualifications
- Minimum one year of clinical social work experience in a healthcare setting.
- Master’s degree in social work (MSW).
- Licensure as an Independent or Graduate Social Worker in the primary work state at hire, with additional state credentials obtained post-hire.
Preferred Skills
Experience with Windows-based software and knowledge of community resources and regulatory issues in the Mid-Atlantic area.
