About the role
This position is responsible for collaborating with the healthcare team to coordinate appropriate, cost-effective services aligned with members' plans of care. The goal is to help members achieve their optimal level of independence and improve quality of life.
Responsibilities
- Conduct psychosocial assessments, identify problems, provide financial counseling, access community resources, and support legal processes.
- Manage and coordinate caseloads, develop and monitor care plans to promote member functioning and life quality.
- Advocate for appropriate resources, ensure timely implementation, and document activities in compliance with standards.
- Analyze outcomes to improve program quality and cost-effectiveness.
- Assist members with application processes for government programs and refer to community agencies as needed.
- Provide counseling and education to members, families, caregivers, and healthcare professionals, including end-of-life issues and directives.
- Maintain current knowledge of laws, regulations, and standards, and coordinate family meetings and guardianship paperwork.
- For inpatient roles, complete assessments required for long-term care as per Medicaid requirements.
Requirements
- Minimum one year of clinical social work experience in a healthcare setting.
- Master’s degree in social work (MSW).
- Credentials from multiple states; primary state credentials required at hire, additional credentials post-hire.
- Licensed Independent Social Worker (DC) or Licensed Master Social Worker (DC, MD, VA) within six months of hire.
Preferred Qualifications
- Experience with Windows-based computer software.
- Knowledge of community resources and systems within the service area.
- Understanding of regulatory issues in the Mid-Atlantic region.
