About the role
We are seeking a qualified social worker to collaborate with healthcare teams in coordinating cost-effective, appropriate services for members, aiming to improve independence and quality of life.
Responsibilities
- Conduct psychosocial assessments, problem-solving, and provide referrals to community resources.
- Develop and monitor individualized care plans in partnership with healthcare professionals.
- Advocate for access to organizational and community resources, ensuring timely and efficient service delivery.
- Document activities in compliance with organizational standards and analyze outcomes to improve programs.
- Assist with applications for programs like Medicaid and Medicare, and address issues such as patient abuse/neglect.
- Provide supportive counseling and education to members, families, caregivers, and staff, including end-of-life planning.
- Schedule family meetings, complete guardianship paperwork, and offer technology support for court proceedings.
- Perform inpatient assessments and other duties as assigned.
Requirements
- Minimum of one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensing credentials required for multiple states, including timely acquisition of local licenses.
Preferred Qualifications
- Experience with computer software, community resources, and regulatory issues in the Mid-Atlantic region.
- Hospital social work experience.
