Case Manager, Social Work - Inpatient (Full-Time)

About the role

This position involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve their independence and quality of life.

Responsibilities

• Conduct psychosocial assessments
• Manage caseloads and develop, monitor, and adjust care plans
• Advocate for and connect members with community resources
• Document all activities in compliance with regulatory and organizational standards
• Analyze outcomes to identify opportunities for improvement
• Assist members with access to public programs
• Provide counseling, education, and support to members, families, caregivers, and healthcare professionals
• Schedule and facilitate family meetings
• Complete guardianship paperwork and facilitate court attendance when needed
• In inpatient settings, complete specific assessments as required

Requirements

• Master’s degree in social work
• At least one year of clinical experience
• Relevant social work licensing credentials in multiple states
• Must obtain specific licenses within six months of hire

Skills

• Proficiency with Windows-based software
• Knowledge of community resources and support services
• Familiarity with regional regulatory and compliance issues
