Case Manager, Social Work - Inpatient (Full-Time)

About the role

This role offers a $7,500 sign-on bonus and is responsible for coordinating appropriate, cost-effective services for members of a defined population, working with the healthcare team to promote independence and improve quality of life.

Responsibilities

• Performing psychosocial assessments, problem-solving, financial counseling, accessing community resources, and supporting legal processes.
• Managing caseloads, developing and monitoring care plans to optimize member functioning.
• Advocating for and facilitating access to community and organizational resources.
• Documenting activities per standards, analyzing outcomes for quality improvement.
• Assisting with applications for public programs and addressing suspected abuse/neglect cases.
• Providing counseling and education to members, families, caregivers, and healthcare teams, including end-of-life discussions.
• Maintaining current knowledge of social work standards, laws, and regulations.
• Scheduling family meetings, completing guardianship paperwork, and providing tech support for court attendance as needed.
• In inpatient settings, completing assessments for long-term care eligibility.

Qualifications

• Minimum one year of clinical social work experience in healthcare.
• Master’s degree in social work (MSW).
• Credentials from multiple states required, with specific licensing timelines for each region.

Skills

• Experience with Windows-based software.
• Knowledge of community resources and regulatory issues in the service area.
