Case Manager, Social Work - Inpatient

About the role

Provides case management for a defined population by collaborating with healthcare teams to coordinate cost-effective, appropriate services that support members' independence and quality of life.

Responsibilities

• Conducts psychosocial assessments, identifies problems, and provides counseling, referrals, and resource access.
• Develops and monitors individualized care plans in collaboration with healthcare professionals to promote optimal functioning.
• Advocates for community and organizational resources, ensuring timely and efficient implementation.
• Documents activities in compliance with standards and analyzes outcomes to improve program effectiveness.
• Assists members with applications for public programs, reports suspected abuse/neglect, and offers education on end-of-life issues.
• Maintains current knowledge of laws, regulations, and standards, and facilitates family meetings and court-related activities.
• Performs additional duties as assigned.

Requirements

• Minimum one year of clinical social work experience in healthcare.
• Master’s degree in social work (MSW).
• Licensure in relevant states, with credentials from multiple states required post-hire.
• For inpatient roles: completion of Uniform Assessment Instruments form for long-term care as per Medicaid requirements.

Skills

• Experience with Windows-based software and knowledge of community resources preferred.
• Understanding of regulatory issues in the Mid-Atlantic region preferred.
