About the role
This role involves coordinating comprehensive social work services within a healthcare setting to support patient independence and quality of life.
Responsibilities
- Conducting psychosocial assessments, problem identification, and providing psychosocial support to members, families, and healthcare teams.
- Developing and monitoring individualized care plans in collaboration with healthcare professionals.
- Facilitating access to community resources, legal processes, and financial counseling, ensuring cost-effective service delivery.
- Documenting activities in compliance with organizational standards and analyzing outcomes for continuous improvement.
- Assisting with applications for public assistance programs and addressing issues like abuse or neglect.
- Providing education and supportive counseling, including end-of-life planning and advanced directives.
- Scheduling family meetings, completing guardianship paperwork, and supporting virtual court attendance as needed.
- Performing inpatient assessments and other duties as assigned, including cross-training to cover multiple locations.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensure from multiple states, with specific credentials required within six months of hire.
- Licensed Independent or Graduate Social Worker in the District of Columbia, and Licensed Master Social Worker in Maryland and Virginia.
Skills
- Experience with computer software in a Windows environment.
- Knowledge of community systems, resources, and regional regulatory issues.
