About the role
This role involves coordinating comprehensive case management services for members of a defined population.
Responsibilities
- Conduct psychosocial assessments, identify problems, and provide support to members, families, and healthcare teams.
- Develop and monitor personalized care plans to promote independence and improve quality of life.
- Facilitate access to community resources, legal processes, and financial counseling.
- Document activities in compliance with organizational standards and analyze outcomes for continuous improvement.
- Assist with application processes for public programs and address issues like abuse or neglect.
- Provide supportive counseling, education, and promote awareness of current case management practices.
- Coordinate family meetings, complete guardianship paperwork, and support virtual court attendance.
Requirements
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licenses required in multiple states within specified timeframes.
Preferred Qualifications
- Experience with computer software, community resources, regulatory issues, and emergency department settings.
