Case Manager - Social Work - Inpatient (Part-Time)

About the role

This role involves coordinating comprehensive case management services for members of a defined population.

Responsibilities

• Conduct psychosocial assessments, identify problems, and provide support to members, families, and healthcare teams.
• Develop and monitor personalized care plans to promote independence and improve quality of life.
• Facilitate access to community resources, legal processes, and financial counseling.
• Document activities in compliance with organizational standards and analyze outcomes for continuous improvement.
• Assist with application processes for public programs and address issues like abuse or neglect.
• Provide supportive counseling, education, and promote awareness of current case management practices.
• Coordinate family meetings, complete guardianship paperwork, and support virtual court attendance.

Requirements

• Minimum one year of clinical social work experience in healthcare.
• Master’s degree in social work (MSW).
• Licenses required in multiple states within specified timeframes.

Preferred Qualifications

• Experience with computer software, community resources, regulatory issues, and emergency department settings.
