Case Manager - Social Work (Inpatient, Part-Time)

Responsibilities

• Conduct psychosocial assessments
• Manage caseloads and develop and monitor care plans
• Facilitate access to community resources and government programs
• Provide counseling, education, and support to members, families, and healthcare professionals
• Ensure accurate documentation and analyze outcomes for continuous improvement
• Schedule family meetings and complete guardianship paperwork
• Support virtual court attendance as needed
• For inpatient roles, complete specific assessments

Requirements

• At least one year of clinical social work experience
• Master’s degree in social work
• Relevant licensure or credentialing in multiple states

Preferred Skills

• Proficiency with Windows-based software
• Knowledge of community resources
• Familiarity with regional regulatory issues
