About the role

This position involves collaborating with healthcare team members to coordinate cost-effective, appropriate services aligned with members' care plans, aiming to optimize independence and improve quality of life.

Responsibilities

  • Conduct psychosocial assessments, identify problems, provide financial counseling, access community resources, and support legal processes.
  • Manage and coordinate caseloads, evaluating patient goals, support systems, health status, and environment to reduce unnecessary medical utilization.
  • Develop and monitor care plans with the healthcare team to promote optimal functioning and quality of life.
  • Advocate for and facilitate access to community and organizational resources efficiently and cost-effectively.
  • Maintain accurate documentation of activities, ensuring compliance with standards.
  • Analyze outcomes to improve program quality and cost-effectiveness.
  • Assist members with applications for programs like Medicaid, Medicare, and Disability, and refer suspected abuse cases to appropriate agencies.
  • Provide counseling, education, and support on end-of-life issues, Advanced Directives, and other topics.
  • Stay updated on social work standards, laws, regulations, and policies, adhering to the NASW Code of Ethics.
  • Coordinate family meetings, complete guardianship paperwork, and assist with virtual court attendance as needed.
  • For inpatient roles, complete Medicaid-required assessments.

Requirements

  • Minimum of one year of clinical social work experience in healthcare.
  • Master’s degree in social work (MSW).
  • Credentials from multiple states required, including licensure as an Independent Social Worker or Graduate Social Worker within six months of hire, depending on location.

Preferred Qualifications

  • Experience with Windows-based software programs.
  • Knowledge of community systems and resources in the service area.
  • Understanding of regulatory issues in the Mid-Atlantic region.

Pay

$7,500 sign-on bonus.
