Social Worker - Case Manager Full Time

About the role

Join an interdisciplinary team as a Full-Time Social Worker - Case Manager. This role involves assessing, planning, advocating, and coordinating patient care from admission to discharge, ensuring compliance with the Case Management Model.

Responsibilities

• Conduct discharge planning assessments within 24 hours of admission and ongoing evaluations.
• Participate in multidisciplinary meetings to address barriers and facilitate timely discharges.
• Educate patients and families on discharge plans and post-acute needs.
• Manage authorization for post-acute services and document all interactions.
• Support ethical decision-making, advance directives, and patient rights.
• Delegate tasks to unlicensed team members and provide mentorship.
• Deliver psychosocial assessments, advocate for high-risk patients, and coordinate community resources.
• Handle cases involving abuse, neglect, domestic violence, and at-risk populations.
• Ensure discharge plans are safe, coordinated, and meet regulatory standards.

Requirements

• Master’s degree in Social Work.
• LICSW or LCSW licensure.
• At least 3 years of social work experience.
• Current BLS certification.

Qualifications

Preferred experience in acute care and case management certification.
