Social Worker Case Manager - Full Time (40 hours/week)

Job Description

The Social Worker Case Manager collaborates as part of a multidisciplinary team to initiate and support discharge planning from the time of admission, in compliance with Centers for Medicare & Medicaid Services (CMS) regulations. Responsibilities include:

• Assessing psychosocial needs
• Identifying barriers to discharge
• Coordinating community resources
• Managing patient cases
• Maintaining timely documentation in the electronic medical record

Qualifications

• Bachelor’s of Social Work (BSW) degree from an educational program accredited by the Council on Social Work Education (CSWE)
• BLS/CPR Certification within 90 days of hire

• Master’s of Social Work (MSW) degree from an educational program accredited by the Council on Social Work Education (CSWE)
• At least one year of previous hospital case management experience
• Case Management Certification
• Rural healthcare experience
• Experience with Cerner EHR

Additional Information

• Wage: Wage range starts at $37.47/hour and goes up to $56.79/hour. Actual wage will be based on years' of applicable experience.
• FTE: 1.0 = 40 hours/week
