Responsibilities
- Provides psychosocial assessments on an as-needed basis, in accordance with Case Management Department Policies and Procedures.
- Promotes and adheres to the IRMC core values of ICARE (Integrity, Compassion, Accountability, Respect, and Excellence) and G.R.E.A.T. (greet/respond/explain/ask/thank) customer standards.
- Utilizes brief interventions and supportive solutions-focused counseling techniques for identified issues.
- Works collaboratively with multiple disciplines within the hospital setting and community.
- Provides crisis intervention whenever appropriate.
- Considers the special needs and behaviors of specific age groups.
- Understands the continuum of care in the healthcare system and possesses the communication skills to interpret the system to patients, families, and other members of the healthcare team.
- Understands third-party payor systems as they relate to discharge planning.
- Utilizes basic computer skills.
Qualifications
- BS in health-related field.
- Graduate curriculum (leading to a master’s degree) in an accredited school of social work preferred or has the documented equivalent in education, training, and/or experience.
- Social work experience in an acute care setting preferred.
